La autora presenta aquí la terminación de un análisis que, si bien trata del caso particular de una paciente transexual, puede interesar a todos los analistas, en particular a aquellos cuyos pacientes tienen necesidad de integrar partes denegadas y clivadas de sí mismos. Simón, aproximadamente a los 20 años, se había hecho operar para cambiar de sexo y se había llamado desde entonces Simona. Una veintena de años más tarde Simona había solicitado iniciar un análisis. En un primer artículo la autora presentó los tres primeros años de este análisis y mostró cierta vacilación en iniciar esta cura. El análisis duró siete años y su terminación permite ahora aclarar a la vez problemas generales planteados por los pacientes que la autora llama heterogéneos. En lo que concierne al transexualismo, este análisis de una paciente transexual operada es, según nuestro conocimiento, la única publicada hasta la fecha, y la calidad de vida obtenida al fin del análisis por la paciente parece satisfactoria. La
paciente tiene ahora un sentimiento de una unidad interna que no se basa solamente en la realidad corporal. Su transexualismo se estableció progresivamente
sobre la base de una diferenciación de sexos así como sobre la estima de un sexo por el otro, y ya no sobre la confusión de sexos y el odio de un sexo por el otro. En lo que concierne el punto de vista más general, esta terminación mostró cómo la disminución de las angustias esquizoparanoides permite integrar partes clivadas y evacuadas, y cómo, con la elaboración de la posición depresiva, los pacientes toleran mejor sus contradicciones internas reencontrando un sentimiento de cohesión del self. Temen menos volverse locos.

The author presents the termination of an analysis which, although it concerns the particular case of a transsexual patient, it can interest other analysts, specially those whose patients need to integrate denied and split parts of themselves. Simon, about twenty years old had himself operated on to change sex and therefore was named Simone. About
twenty years later, Simone asked for an analysis. In an article published in 1998, the author presented the first three years of this analysis and expressed her hesitations in taking it on. The termination of this seven-year analysis clarifies problems that touch both transsexual patients and, in a more general fashion, patients that the author calls heterogeneous. In the area of transsexualism, here we have, to our knowledge, the first publication of a transsexual, operated on patient’s analysis. The quality of life that the
patient obtained at the end of the analysis is satisfying. Simone, now has a feeling of internal psychic unity that is no longer founded on external corporal appearance. Since then her transsexualism has been established on the basis of sexual differentiation and of esteem of one sex for the other, and no longer on the basis of a sexual confusion or reciprocal hatred. In a more general point of view, the termination has shown how the diminishing of schizo-paranoid anxieties allows to reintegrate split and evacuated parts
of the self; it has also shown how, with the work-through of the depressive position, patients tolerate their internal contradictions better, finding again a feeling of cohesion of the self. They are less afraid of becoming mad.

L’auteur présente ici la terminaison d’une psychanalyse qui, bien qu’elle concerne le cas particulier d’une patiente transsexuelle, peut intéresser tous les analystes, en particulier ceux dont les patients ont besoin d’intégrer des parties déniées et clivées d’eux-mêmes. Simon, à l’âge de 20 ans environ, s’était fait opéré pour changer de sexe et s’était désormais appelé Simone. Une vingtaine d’années plus tard Simone avait demandé d’entreprendre une psychanalyse. Dans un premier article l’auteur a présenté les trois premières années de cette analyse et montré ses hésitations à entreprendre cette cure. L’analyse a duré sept ans et sa terminaison permet maintenant d’éclairer à la fois des problèmes spécifiques aux patients transsexuels, mais aussi des problèmes généraux posés par les patients que l’auteur appelle hétérogènes. En ce qui concerne le transsexualisme, cette psychanalyse d’une patiente transsexuelle opérée est, à notre connaissance, la seule qui ait été publiée à ce jour, et la qualité de vie obtenue en fin d’analyse par la patiente semble satisfaisante. La patiente a maintenant un sentiment d’une unité interne qui n’est plus basée uniquement sur la réalité corporelle. Son transsexualisme s’est progressivementé tabli sur la base d’une différenciation de sexes ainsi que sur l’estime d’un sexe pour l’autre, et non plus sur la confusion des sexes et la haine d’un sexe pour l’autre. En ce qui concerne le point de vue plus général, cette terminaison a montré comment la diminution des angoisses schizo-paranoïdes permet de réintégrer des parties clivées et évacuées, et comment, avec l’élaboration de la position dépressive, les patients tolèrent davantage leurs contradictions internes en retrouvant un sentiment de coh ésion du self. Ils craignent moins de devenir fous.
